  • Complete application form (receivable upon payment of $50 non-refundable application fee)
  • Background check (fee included in the application fee)
  • 2 letters of reference, 1 personal and 1 professional 
  • Current resume
  • Current professional photo (taken from the waist up)
  • Copy of academic transcript(s) illustrating collegiate coursework, if applicable
  • Copy of professional license(s), if applicable
